NEW MEDICAL CONSENT FORM REQUIRED
Idaho Legislation passed a new law adding an amendment to Chapter 10, Title 32, Idaho Code, regarding “Parental Rights in Medical Decision Making.”
The amendment states:
An individual shall not furnish care services or ask to furnish a health care service to a minor child without obtaining consent of the minor’s parent or legal guardian. Health care services include the diagnosis, screening, examination, prevention, treatment, cure, care, or relief of any physical or mental health condition, illness, injury, defect, or disease.
Therefore, we cannot discuss any medical or mental health issues with the youth in our custody without a parent or legal guardian signing the new medical consent form. This also includes assisting your child with any prescribed medications.
Please contact us for any questions regarding the new medical consent form so we can care for your child’s medical and mental health while they are in our custody.
The new law also requires that the biological parent or adoptive parent sign the new medical consent form. If you are the legal guardian of a child in our custody, you must have the court documentation stating that you have the legal authorization to make any and all medical decisions for that child.
Thank you,
Snake River Juvenile Detention Center
FILLING PRESCRIBED MEDICATIONS
If your child is taking any type of prescribed medication please notify the Snake River Juvenile Detention Staff as soon as possible. There are two ways that medications can be accepted into the Facility for your child.
- Contact the prescribing doctor’s office to call the prescription for medications into Kurt’s Pharmacy or Dick’s Pharmacy to be filled. Kurt’s Pharmacy and Dick’s Pharmacy will then deliver the medications to the Facility once they have been paid for.
- It is important that you tell the pharmacy your child is at the Snake River Juvenile Detention Center.
Dick’s Pharmacy
526 Shoup Ave W Ste K
Twin Falls, ID 83301
(208) 734-7373
Kurt’s Pharmacy
Lynwood Shopping Center
1203 Filer Ave E
Twin Falls, ID 83301-4118
(208) 734-8177
- You may also take your child’s medications to your own pharmacy and have them bubble pack a 7-day supply of the medications. Medications will only be accepted in an authorized bubble pack with all information regarding the medications. Once the medications have been properly bubble packed by a pharmacy you may bring them to the facility for your child.
- If your child utilizes the use of an inhaler, please bring the inhaler to the Facility immediately. While in the Facility your child does have access to a rescue inhaler but if used you will be responsible to reimburse the Facility for the cost of the inhaler.
- When your child is released from the Facility the remaining medications in the bubble pack or inhaler will be released back to you at the time of your child’s departure.
NOTE: If you haven’t signed the new medical consent form yet, you will be receiving the form in the mail. This is very important for your child’s welfare, in case medical attention is needed in the event of a medical emergency. Please sign this form and return it to us as soon as possible. You will be notified immediately or as soon as possible if your child does have a medical emergency. You may also sign one of these forms at the front desk if you are at the facility.
