Grievance Form
As a recipient of victim support services through the Youth Services Program, you have the right to file a written grievance if you believe you have been treated unfairly in any procedure, action, or program/activity. Please complete the following questions as in-depth as possible and return to Kevin Sandau, Chief Probation Officer/Director, for review. You can also file a grievance with the Idaho Council of Domesti Violence and Victim assistance on their website:https://icdv.idaho.gov/resources/complaints.
